Primary Care at YourTown Health

Primary care is where good health starts, and at YourTown Health it is built around your whole family. Our physicians, physician assistants, and nurse practitioners provide high-quality care for patients at Community Health Centers across Meriwether, Pike, Lamar, Carroll, Coweta, and South Fulton counties, regardless of your insurance status.

Preventive Care and Everyday Health Needs

Regular visits make it easier to stay on top of your health. Your primary care team provides annual physicals, cancer screenings, immunizations, and diagnostic labs, and treats short-term illnesses when they come up. A provider who knows your history can help you plan each next step with confidence.

Looking for care for a child or teenager? Our Pediatric and Adolescent Care team sees patients from newborns through age 18. Our Women’s Care team supports annual exams, screenings, and prenatal care.

Support for Managing Chronic Conditions

If you live with diabetes, high blood pressure, or asthma, your primary care provider can help you build a plan that fits your daily life. Our providers take a whole-person approach, looking at your lifestyle and goals as well as your numbers. YourTown Health also takes part in the National Health Disparities Collaborative, which compares our results for hypertension, diabetes, cardiovascular disease, asthma, and cancer care with national averages.

On-Site Lab Testing with Timely Results

Many common tests can be done during your visit, including urine pregnancy tests, rapid strep tests, blood glucose checks, routine urinalysis, and flu, PCR, and RSV testing. Most other lab results are back in about a day or so.

Vaccines for Children and Adults

Our offices provide childhood vaccines on the recommended schedule through age 19, along with adult vaccines such as flu, tetanus, and Tdap. Hepatitis A and B, pneumococcal, varicella, and HPV vaccines are also available. Not sure what you or your child are due for? Ask at your next visit and your care team will walk through it with you.

Geriatrics

Health needs change as we age. Our geriatric care focuses on preventing, diagnosing, and treating age-related conditions so older adults feel supported at every stage. For everyday ideas, see our fitness tips for seniors.

Referrals to Trusted Specialists

When you need care beyond primary care, we refer you to qualified specialists. Many local specialists offer a sliding fee scale, which can help keep that next step affordable.

Affordable Care for Every Family

YourTown Health is a Federally Qualified Health Center, and we welcome patients whether or not they have insurance. We accept Medicaid, Medicare, and many private plans. If you are uninsured, or need help with copays, our Sliding Fee Scale bases your costs on household income and household size.

What is the difference between primary care and QuickCare?
Primary care is for ongoing and preventive health, such as annual physicals, screenings, and managing conditions like diabetes. QuickCare is our walk-in clinic in Palmetto for common, non-life-threatening illnesses and minor injuries when you need care without an appointment. For life-threatening emergencies, call 911.
